Thank you for choosing our dental practice for your care. We are committed to your treatment and experience being successful and pleasant. Please understand that payment of your bill is considered a part of your treatment.

Major Service Arrangements

Our office may require a deposit against the total charges for a major procedure. For implants we require the balance paid in full before placement.

Insurance Company Verification and Rejected Claims

Please remember your insurance policy is a contract between you and your insurance company. As a courtesy we file primary insurance, we do not file secondary. All dental insurance coverage needs to be verified before any services are rendered. In the event of an emergency the patient is responsible for the emergency treatment, and the insurance company will reimburse the patient. Note: Verification of insurance is NOT a guarantee of coverage: If the insurance company pays less than the estimated amount then the patient is responsible for the remaining balance. You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates. If a claim for services is rejected by the insurance company due to plan exclusions, clauses, waiting periods, cancellation of coverage, or any other reasons, the balance will be the patient's responsibility. Please remember for any service that is not covered our practice charges what is usual and customary for our area.

Bad Debt/ Legal Action

If an account is not paid in full or satisfactory arrangements are not made within the allowable time, the dental office reserves the right to refer the account to an attorney and/or collection agency for collection of the balance. In the event the account is sent to collection, their fees will be added to the delinquent balance.

Appointment Cancellations

This valuable time is reserved to provide you with quality dental care. We make every effort to honor all time commitments and request that you extend the same courtesy to us. Unless cancelled at least 48 hours in advance, our policy is to charge for missed appointments at a minimum of 30.00. Please help us serve you better by honoring scheduled appointments.
